Purbaya Preps Rp20tn to Clear BPJS Health Insurance Arrears

TEMPO.CO, Jakarta - The Indonesian government plans to forgive accumulated premium arrears for specific categories of participants enrolled in BPJS Kesehatan (Social Security Agency for Health). Finance Minister Purbaya Yudhi Sadewa has allocated a substantial budget for this debt relief initiative.

The funds will be allocated by the state treasury for the next fiscal year. Purbaya also urged BPJS Kesehatan to improve its operational management. "We have budgeted Rp20 trillion. We're asking BPJS Kesehatan to also improve its management," he stated at the Coordinating Ministry for Economic Affairs office in Central Jakarta on Wednesday, October 22, 2025.

Target Recipients of Arrears Forgiveness

BPJS Kesehatan President Director Ali Ghufron Mukti elaborated on the debt forgiveness plan. He explained that the premium forgiveness will target independent participants who have transitioned to become Premium Assistance Recipients (PBI).

PBI participants in the National Health Insurance (JKN) scheme are low-income individuals whose premiums are fully covered by the central or regional government.

"So, the essence of this amnesty is for people who have switched categories, for example, those whose contributions were previously independent and self-paid, but who fell into arrears before switching to PBI," Ali said at the Ministry of Finance office on Wednesday, October 22, 2025.

He added that the forgiveness facility will also be available to Non-Wage Worker (PBPU) participants whose contributions have been covered by the local government. The government is striving to ensure that the debt forgiveness is properly targeted using decile measures and the National Social and Economic Single Data (DTSEN).

The Scale of the Debt

There is an option to extend a similar facility to Class 3 independent participants, but a final decision on that matter has not yet been made. Ali stressed that while BPJS Kesehatan wants participants to access necessary services, they should not misuse the system. "Those who can afford it must pay," he affirmed.

Ali previously noted that 23 million participants are currently in arrears on their contributions, with a total debt exceeding Rp10 trillion. The government is preparing this amnesty scheme so that participants who genuinely cannot afford their old debt can reactivate their membership without the burden of historical obligations.

"It's clearly more than Rp10 trillion. Previously, it was Rp7.6 trillion, but that didn't include other things," Ali said, as quoted by Antara.
